1) Access the GoLookUp opt-out page and fill out the online form

Go to the GoLookUp opt out page. Fill out all of the fields on the opt-out form except for the date of birth. All other information is required. Do not enter your DOB as it is considered sensitive data. 

2) Submit the opt-out request

3) Continue your data removal journey
