Go to https://www.numlookup.com/opt_out. Enter your phone number, solve the CAPTCHA, and click “Remove My Info”
Go to the NumLookup opt-out page. Enter the full phone number you want to be removed, complete the “I’m not a robot” CAPTCHA challenge, and click “remove my info.”

Choose your removal method (manual removal request recommended)
If you have a NumLookup account, sign in. If you don’t have an account, you can sign up (not recommended) or click the “manually submit removal request” button.

Fill in the opt-out form
Make sure to fill in their opt-out form with the most up-to-date information. You’ll need to provide:
– Your email address
– Your full name
– Your street address
– Your city and state
– Your phone number
– Subject (Remove my data)

Solve the CAPTCHA, and click “Submit” to complete your removal request
